Billing/ Payment Policies

We expect patients visiting us for medical treatment to pay their co-pay at the time of service. Our office requires payment at the time of service for cosmetic visits as well.

Visa, MasterCard, American Express and Discover card are acceptable forms of payment.

Referrals

If you have an insurance that requires a referral, please contact your Primary Care Physician to ensure the referral is done prior to your appointment. Failure to contact the physician makes you financially responsible for the office visit.

If your referral is not received before your office visit, you will be required to sign a waiver which would allow us to bill you for the services in the event your Primary Care Physician does not give you a referral for the visit. We are sorry, but if referrals are not received, you will be billed.
